SUMMARY OF
A.H. BENEFITS PROGRAMS
The following Benefits Summary is not a legal document, rather a brief summary for general information purposes. All benefits are governed by and administered according to the legal documents of each plan. A.H. reserves the right to modify, suspend, and discontinue the plans at any time.


Fulltime employees are eligible for the following benefits on the 1st of the month following their 90th day of employment: Medical Plans, HMO, PPO , Dental Insurance , Life Insurance , Premium Only Plan (POP) , Short Term Disability (STD) , Long Term Disability (LTD) , Meadows Federal Credit Union , Holidays , Attendance Bonus Program.

In addition to the above benefits fulltime employees are eligible for the following benefits after one full year of employment: Vacation, Retirement and Profit Sharing.


Medical Plans

A.H. Entertainers, Inc. offers a choice of two health insurance plans through United Healthcare. The majority of this cost is paid by the company. The options available to you and your dependents are as follows:

HMO
(Health Maintenance Organization)


As a member of the HMO, you choose a doctor from a provider directory. This doctor becomes your primary care physician. You must go to this doctor for all of your health care needs. You can only see another doctor if you are referred. There are no deductibles or claim forms. You pay $25.00 per doctor office visit and the plan covers your medical expenses at 100%. Prescription drug care is also provided (see plan description for more details)

PPO
(Preferred Provider Organization)


As a member of the PPO, you are covered at a different rate depending upon whether you use a participating or nonparticipating doctor. $200 yearly deductible per person and $600 max per family is included. The plan covers $15 copay for participating doctor office visits, then 80% for most services provided by a participating doctor, and 60% for nonparticipating doctors after yearly deductible is met. Prescription drugs care is also provided (see plan description for more details)


Dental Insurance


A.H. Entertainers, Inc. offers two dental programs. DMO (Dental Maintenance Organization) covers all preventative care at no cost as long as office visits are scheduled six months apart, and Out-Of-Network offers coverage at various levels for any dentist the employee wishes to visit. Further details are available in the plan description. Employees pay the full cost of dental insurance.


Life Insurance

A.H. Entertainers, Inc. offers life insurance coverage with options to select coverage for spouse and dependent children. Employees may elect up to $200,000 in coverage, up to $100,000 for their spouse, and $5,000 in coverage for a dependent child(ren). Eligible employees who elect Group Term Life Insurance are responsible to pay 100% of the premium cost. Employee Life Insurance provides protection of life insurance at age-related group rates. Contact Human Resources for individual life insurance rates based on total coverage elected.

Premium Only Plan (POP)

The Premium Only Plan allows employee payroll deductions for group insurance premiums to be taken before taxes instead of after taxes. The POP plan is available for group health, dental, and term life insurance plans.



Short Term Disability (STD)

STD insurance is offered as income protection in the event of an illness or injury that causes disability. STD pays 60% of your salary up to a maximum of $250/week for up to 13 weeks. Employees pay the full cost of STD insurance.

Long Term Disability (LTD)

LTD insurance is offered as income protection in the event of an illness or injury that causes disability. LTD pays 60% of your salary up to a maximum of $4,000/month. The waiting period for LTD is 90 days for both illness and injury. Employees pay the full cost of LTD insurance.

Meadows Federal Credit Union

Employees may have a portion of their paycheck directly deposited in a checking and/or savings account offered at Meadows Federal Credit Union. Please contact Human Resources for further details.

Holidays

A.H. provides 6 paid holidays a year including New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, and Christmas Day. (See A.H. Policy Manual for further details.)

Attendance Bonus Program

Paid time off is accrued on a fiscal year basis from January to December. After the first full year of service, employees can earn a total of five days paid absence. The company pays employees an Attendance Bonus for the balance of unused time off in each fiscal year on or about March 15 following the year in which paid time off was earned. (See A.H. Policy Manual for further details).

Fulltime employees are eligible for the following benefits after one full year of employment.


Vacation

Vacations are accrued or earned based on the employee's anniversary date of hire and on the time worked during the preceding anniversary year. Upon completion of one anniversary year of employment, employees may use the vacation hours they have accumulated. The following outlines paid vacation time earned in relation to length of service:

After 1 full year of service.............5 days
After 2 full years of service.........10 days
After 8 full years of service.........15 days

Retirement and Profit Sharing

A.H. Entertainers, Inc. offers a 401(k) plan to eligible employees in January or July following 1 full year of service. The profit sharing plan provides a distribution into the 401(k) plan based on business performance. (See plan description for further details)
